This Privacy Policy explains what information Forma Motorsport collects when you use our website, request an estimate, schedule a repair, or contact us — and how we use, share, and protect that information. It also explains your rights, especially if you live in California.

1. Who we are

"Forma Motorsport," "we," "us," and "our" refer to Forma Motorsport, an auto body and collision repair business operating two locations:

2. Information we collect

Information you give us directly

When you fill out a form on our website, call us, email us, or visit one of our shops, you may give us:

• Contact information — name, phone number, email address
• Vehicle information — year, make, model, license plate, VIN, photos of damage
• Incident details — how the accident happened, location of damage, drivability
• Insurance information — carrier name, policy number, claim number, adjuster details
• Attorney status — whether you have legal representation or are looking for one
• Preferences — preferred contact time, rental car needs
• Payment information — collected at the shop when you authorize the repair; not stored on our website

Information collected automatically

When you visit our website, our hosting provider may automatically collect standard server log data such as IP address, browser type, pages visited, and timestamps. We do not currently use Google Analytics, Meta Pixel, or other third-party advertising trackers on our site.

3. How we use your information

We use the information we collect to:

• Respond to your estimate request and schedule your repair
• Communicate with you about your appointment, repair status, and pickup — by phone, SMS, and email
• Coordinate directly with your insurance carrier on your behalf
• Arrange complimentary towing and rental car coordination when applicable
• Refer you to a personal injury attorney if you tell us you're looking for one
• Send follow-up requests for reviews after your repair is complete
• Comply with legal obligations and resolve disputes
• Improve our services

4. SMS and text messaging

If you provide us with your mobile phone number, you agree we may contact you by phone call and text message (SMS) regarding your estimate, appointment, repair status, and follow-up — including via automated systems. Message frequency varies. Standard message and data rates may apply.

You can opt out of SMS communications at any time by replying STOP to any text we send. Reply HELP for assistance. Consent to receive text messages is not a condition of any purchase. Opting out of SMS will not affect our ability to contact you by phone or email about an active repair.

5. Who we share your information with

We share your information only with the following categories of third parties, and only for the purposes described:

• Insurance carriers and adjusters — to coordinate your claim and authorize repairs on your behalf
• Tow operators and rental car partners — when arranging logistics for your repair
• Personal injury attorneys — only when you have specifically told us you are looking for legal representation and have asked us to refer you
• Service providers we use to run our business, including:
  • GoHighLevel (GHL) — CRM and communications platform that stores your contact record and sends our follow-up emails and SMS
  • Netlify — website hosting and form-submission storage
  • Carwise — online photo-estimate and booking widget on our website
  • Google — Google Maps embeds and Google Workspace email (info@formamotorsport.com)
• Law enforcement, courts, or regulators — when required by valid legal process

We do not sell your personal information for money, and we do not share it with third-party advertisers for cross-context behavioral advertising.

6. How long we keep your information

We keep your information for as long as needed to provide services and meet legal obligations — typically the longer of (a) the duration of the repair plus 7 years for tax, warranty, and insurance recordkeeping, or (b) any applicable statute-of-limitations period under California or Arizona law.

7. How we protect your information

We use reasonable administrative, technical, and physical safeguards designed to protect your information. No method of transmission or storage is 100% secure, however, and we cannot guarantee absolute security.

8. Your California privacy rights

If you are a California resident, the California Consumer Privacy Act (CCPA) and the California Privacy Rights Act (CPRA) give you the following rights:

• Right to know what personal information we have collected about you and how we use and share it
• Right to delete personal information we have collected, subject to certain exceptions (e.g. we may need to keep records for tax, warranty, or legal compliance)
• Right to correct inaccurate personal information
• Right to opt out of the "sale" or "sharing" of personal information — though we do not sell or share your information for cross-context behavioral advertising
• Right to limit the use of sensitive personal information
• Right to non-discrimination — we will not deny you services, charge you a different price, or provide a lower quality of service because you exercised any of these rights

To exercise any of these rights, email info@formamotorsport.com with the subject line "California Privacy Rights Request," or call (888) 899-5682. We will verify your identity before responding and aim to reply within 45 days, as required by law.

9. Children's privacy

Our website and services are not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child has provided us with information, please contact us and we will delete it.

10. Changes to this policy

We may update this Privacy Policy from time to time. When we do, we will update the "Last revised" date at the top of this page. Material changes will be noted on our homepage or in an email to active customers.
