Terms of Service

These Terms of Service govern your use of the Forma Motorsport website and the auto body, collision repair, paint, vehicle wrap, paint protection film, body kit installation, and towing services we provide. By using our website or authorizing us to perform work on your vehicle, you agree to these terms.

1. About us

Forma Motorsport is an auto body and collision repair business with two locations: Los Angeles, CA (14600 Titus Street, Panorama City, CA 91402) and Phoenix, AZ (11210 N Cave Creek Rd, Phoenix, AZ). Together with our owners, employees, and authorized agents, we are referred to as "Forma Motorsport," "we," "us," or "our."

2. Website use

You may browse our website, request an estimate, schedule a repair, or contact us. You agree to use our website for lawful purposes only and not to:

• Submit false or misleading information
• Attempt to access non-public areas of our systems
• Interfere with the website's operation or attempt to overload it
• Use the website to send unsolicited commercial messages to others

3. Estimates, quotes, and pricing

Free estimates we provide — whether in person, by phone, via photo estimate through Carwise, or by email — are based on the information available at the time and the visible damage we can assess. Final pricing may differ from the initial estimate if additional damage is discovered during teardown or if parts costs change.

Any quote is valid for 30 days from issuance unless otherwise stated in writing. We will obtain your authorization in writing before performing any work beyond the original estimate, as required by California Business and Professions Code § 9884.9 (where applicable to our LA shop).

4. Insurance coordination

If your repair is being paid for by an insurance carrier, we work directly with the carrier and adjuster on your behalf — including writing the estimate, requesting supplements, negotiating coverage, and obtaining authorization. You authorize us to communicate with your insurance carrier and to share information necessary to coordinate your claim.

You remain responsible for any deductible, any work not covered by insurance, and any difference between what your insurance pays and what we charge if that difference exists.

5. Warranty

Forma Motorsport offers a lifetime warranty on paint performed at our facility. This warranty covers paint defects (peeling, fading, delamination) attributable to our workmanship or materials, for as long as you own the vehicle.

The lifetime paint warranty does not cover:

• Damage from collisions, road debris, or other impacts after delivery
• Damage from improper washing, harsh chemicals, abrasives, or commercial car washes
• Damage from environmental causes (fire, flood, bird droppings left unaddressed, prolonged UV exposure without normal care)
• Repairs performed by another shop after our work
• Areas not painted by us
• Pre-existing conditions disclosed before work began

Other services we perform (collision structural work, body kit installation, vehicle wraps, paint protection film) may carry separate manufacturer warranties from the parts manufacturers (e.g. XPEL, 3M, Glasurit). Those terms are set by the manufacturer and provided at the time of service.

6. Payment and storage

Payment is due upon vehicle pickup unless other arrangements have been made in writing or your insurance carrier is paying us directly. We accept cash, major credit cards, debit cards, and certified checks. Personal checks are subject to clearance before vehicle release.

Vehicles left at our facility for more than 7 days after completion of work and notification to the customer may be subject to daily storage fees in accordance with applicable state law. Vehicles abandoned at our facility may be subject to mechanic's lien sale under California Civil Code § 3068 (LA location) or Arizona Revised Statutes § 33-1021 (Phoenix location).

7. Towing and rental coordination

When you commit to a repair with us, complimentary towing is included within a 200-mile radius of either Forma location. If you cancel your repair before work begins after towing has been provided, you may be charged the tow operator's invoice cost.

Rental cars provided through your insurance carrier are governed by your insurance policy. If we coordinate a courtesy rental on your behalf for an uncovered period (up to two weeks for repairs where liability has been accepted and the repair is committed), you remain responsible for any damages to the rental and any charges incurred beyond the courtesy period.

8. Attorney referrals

If you have been in an accident and tell us you are looking for legal representation, we may refer you to a personal injury attorney. Forma Motorsport is not a law firm, does not provide legal advice, and has no attorney-client relationship with you. Any attorney we refer you to is independent of Forma Motorsport, and your relationship with that attorney is governed by your separate agreement with them.

9. Cancellations

You may cancel a scheduled appointment at any time before work begins by calling (888) 899-5682. If work has already begun, you remain responsible for any work performed and any parts ordered specifically for your vehicle.

10. Limitation of liability

To the maximum extent permitted by law, Forma Motorsport's total liability to you for any claim arising out of our services or your use of our website is limited to the amount you paid us for the specific service giving rise to the claim. We are not liable for indirect, incidental, special, consequential, or punitive damages, including lost profits, lost data, or lost use of your vehicle, except where such limitation is prohibited by law.

Nothing in these Terms limits your rights under California or Arizona consumer protection law that cannot be waived by contract.

11. Dispute resolution

We aim to resolve disputes informally first. If you have a concern about a repair or our service, please contact us at (888) 899-5682 or info@formamotorsport.com. We will respond within 5 business days and work in good faith to resolve the issue.

If informal resolution is not possible, any dispute will be resolved in the courts of Los Angeles County, California (for repairs performed at our LA location) or Maricopa County, Arizona (for repairs performed at our Phoenix location), and you and Forma Motorsport agree to the personal jurisdiction of those courts. California residents retain all rights under California state law, including the right to file a complaint with the California Bureau of Automotive Repair.

12. Governing law

These Terms are governed by the laws of the State of California for matters arising at our LA location, and the State of Arizona for matters arising at our Phoenix location, without regard to conflict-of-law principles.

13. Changes to these Terms

We may update these Terms from time to time. When we do, we will update the "Last revised" date at the top of this page. Continued use of our website or services after we post changes constitutes your acceptance of the updated Terms.
